The test type used is the LumiraDx rapid antigen.
Each employee will receive a lab report within 24 hours of their received test sample.
Each employee submits their patient data using the Patient Intake Form to include their insurance information, and their insurance will be billed directly. If your organization collects the test samples from your employees (we will deliver the testing supplies and instructions) there is no cost for this testing to your organization.
If you prefer to have Prevention Healthcare Services come to your location and collect the test samples, it will cost your organization $6 per each employee (still billing each patient’s insurance directly).
If your organization needs COVID-19 testing, follow these simple steps:
1. Fill out and submit the Employer COVID-19 Testing form.
2. Request that each employee fill out and submit the Employee COVID-19 Testing Patient Intake Form. 3. Request that each employee submit their identification and insurance cards via the Upload Secure Documents link found on the website.
